Mechanism of Action
Acetylcysteine works by breaking the disulfide bonds within mucoproteins in the phlegm. This chemical action reduces the viscosity (thickness) of the mucus, making it easier to expel from the respiratory tract through coughing.
Indications
For the treatment of respiratory conditions associated with thick and sticky mucus, such as:
- Acute and chronic bronchitis.
- Productive cough (a "wet" cough).
- Chronic Obstructive Pulmonary Disease (COPD).
- Sinusitis and chest congestion.
Dosage
For Adults and Adolescents from 14 years of age:
- The usual dose is 600 mg per day, which can be taken as one 600 mg capsule once daily, or divided into smaller doses as directed by a physician.
- It is often recommended to take it after a meal. Swallow the capsule whole with plenty of water.
Warnings and Precautions
- Critical Interaction: Do not take with cough suppressants (antitussives). Acetolytic thins mucus, and the cough reflex is essential to clear it. Combining them can lead to a dangerous buildup of secretions.
- Asthma & Bronchospasm: Use with caution in patients with asthma, as it can rarely cause airway narrowing (bronchospasm). Stop use and consult a doctor if this occurs.
- Peptic Ulcers: Use with caution in patients with a history of stomach ulcers.
Side Effects
While generally well-tolerated, side effects can include:
- Uncommon: Nausea, vomiting, diarrhea, or stomach pain.
- Rare: Allergic reactions (rash, itching), headache, or bronchospasm.
Key Patient Advice
- Drink plenty of water throughout the day to help the medication work effectively.
- Remember the "rotten egg" (sulfur) smell is normal for this medication.
- Do not combine with a dry cough medicine.
