Q1What is miliaria?
Also calledheat rash · sweat rash · prickly heat
Causeobstruction or disruption of eccrine sweat ducts
Resultsweat retention within the skin
Lesion typevesicles or papules — depends on the level of ductal involvement
Two formsMC = most superficial · MR = deeper, inflammatory
Settinghot and humid environments
Agesall ages · infants especially susceptible
Q2Where is the block? — the difference that explains everything
Tap a level to isolate it.
MCblock in stratum corneum or immediately beneath → sweat in superficial intraepidermal space → clear vesicles · little or no inflammation
MRblock at mid-epidermis → sweat retention + leakage into surrounding epidermis → inflammatory response → erythematous papules and papulovesicles
Profundasweat retention in the dermis · may occasionally follow repeated MR
Miliaria rubra→ repeated episodes →Miliaria profunda (occasionally)
Memory aid (study addition)
Crystallina = crystal clear · top layer · calm (no inflammation)
Rubra = ruby red · mid-epidermis · itchy
From MC to MR: deeper → redder → itchier → bigger (1–2 mm → 2–4 mm)
Q3How does each one look and feel?
Miliaria crystallina
Lesiontiny, clear, superficial vesicles
Size1–2 mm
Looklike droplets of water or beads of sweat
Rednessno significant erythema
Symptomsusually no itching, burning, or pain
Fragilityrupture easily → fine bran-like scaling
Sitesforehead · scalp · neck · upper trunk
Newbornshead · neck · upper body
Miliaria rubra
Lesionerythematous, non-follicular papules and papulovesicles
Size2–4 mm
Backgroundsurrounding erythema
Symptomsitching · prickling · stinging · burning
Worse withsweating · heat exposure
Sitesneck · upper chest/back · forehead and scalp margins · axillae · groin and folds · under tight clothing
Infantstrunk + folds of neck, axillae, groin
Adultsupper trunk · neck · clothing-friction areas
Miliaria pustulosa
Whatpustular variant of MR — pustules instead of papules
Trappustules may suggest infection → do not assume it without supporting findings
Miliaria profunda
Whatdeeper, skin-colored papules
Sweatingmore significant impairment
Tell MC from MR in 3 questions (study addition)
1 · Clear or red? → clear = MC · red = MR
2 · Silent or itchy? → silent = MC · itchy/prickly = MR
3 · 1–2 or 2–4 mm? → small = MC · larger = MR
Pustulosa = pustules · Profunda = profound (deep)
| Feature | Miliaria crystallina | Miliaria rubra |
|---|---|---|
| Level of obstruction | Stratum corneum or immediately beneath it | Mid-epidermis |
| Primary lesions | Clear superficial vesicles | Erythematous papules and papulovesicles |
| Inflammation | Absent or minimal | Prominent |
| Itching | Usually absent | Common |
| Appearance | Tiny water-like droplets | Small inflamed bumps |
| Common age groups | Particularly newborns | Infants, children, and adults |
| Clinical course | Usually rapidly self-limiting | May persist longer if heat and sweating continue |

Q4Who gets it, and when?
Worldwidemore with excessive sweating, high temperature, humidity
Infantseccrine ducts relatively immature
MC · babiesneonates, first weeks of life · up to ~9% of newborns
MC · adultsfebrile illness or excessive heat
MRmost common form · infants, children, adults · prolonged sweating in hot, humid conditions
MR · classicpeople moving to tropical climates or suddenly exposed to heat
Bedriddenprolonged occlusion + limited ventilation
Q5Why does it happen?
Memory aid (study addition): a SOLID plug blocks the duct
S · Sweatingexcessive sweating: hot weather · high humidity · physical activity · fever
O · Occlusiontight clothing · excessive wrapping · bedding contact · occlusive dressings
L · Local skinprolonged moisture · friction · occlusion
I · Immature ductsexplains why newborns are susceptible
D · Duct blockabnormal keratinization + plugging of duct openings · duct-wall disruption lets sweat escape
1 · Duct blockedobstruction or disruption
2 · Sweat retainedcannot reach the surface
3 · Lesions formvesicles or papules, depending on the level
Q6How is it diagnosed?
Main methodclinical: appearance + distribution + history of heat, sweating, or occlusion
MC findingclear, superficial, fragile vesicles without inflammation
MR findingerythematous non-follicular papules and papulovesicles + itch or prickling
Labsnot required in typical cases
Dermoscopyoptional, in selected patients
Biopsyrarely needed · only for persistent, recurrent, or atypical cases
Q7What can look like it? — clue → think
Neonatal acnepapules and pustules on the cheeks, forehead, nose · follicular
Erythema toxicummacules, papules, pustules with a red halo · starts in the first days of life
TNPMpustules → pigmented macules ± collarette of scale · often present at birth
Folliculitiscentered on hair follicles
Contact dermatitisfollows irritant or allergen exposure · the distribution matches the exposure
Neonatal infectionHSV or bacterial · exclude if fever, poor feeding, lethargy, or rapid spread
Key separator (study addition)
Miliaria = non-follicular · acne and folliculitis = follicular
Q8How is it treated?
Goalsreduce sweating · restore evaporation · calm inflammation
Both formscool, ventilated room · light, loose, breathable fabrics · no over-wrapping · remove wet clothes · treat fever
Avoidheavy, occlusive creams and ointments · excessive washing or rubbing
MCno specific medication · cool the skin · do not squeeze or pop the vesicles
MR · mildcooling + less sweating + no occlusion
MR · itchyshort course of a topical corticosteroid, chosen by a clinician · low-potency for children and sensitive areas
MR · soothingcalamine · cool compresses · cool baths
Infantssteroids cautiously, only on professional advice
Antibioticsnot routine · only if a bacterial infection is clinically diagnosed
Q9Complications and prognosis
Usual courseuncomplicated · no permanent damage
Infectionsecondary bacterial infection, especially after scratching
Recurrencehappens if the heat and sweating continue
Sweatingtemporary impairment → if extensive, risk of heat-related illness
Progressionrepeated MR → occasionally profunda
Prognosisexcellent · MC resolves rapidly · MR may persist longer with continued exposure
Q10When to seek medical assessment
Rashwidespread, persistent, or worsening
Lesionspainful · markedly inflamed · extensive pustules
Infantfever · poor feeding · lethargy
Uncertaina newborn with vesicles or pustules
Overheatingweakness · altered consciousness · high temperature
